Blank Forms
To book a laboratory/simulation suite/home health suite complete "HSSC Booking" form and to book equipment use "HSSC Equipment Requisition". Complete appropriate form and submit it either in person or online. To submit in person, give the "HSSC Room Booking" form to Faculty of Health Sciences field base staff and the "HSSC Equipment Requisition" to a HSSC technician. Please note that HSSC technicians check all online submitted forms three times a day (0800, 1200 and 1500 hours) Monday to Friday only. As well, the "HSSC Equipment Requisition" is due two working days prior to class.

Standard Requisitions
Standard requisitions are for students and faculty to book equipment. Please complete date, time, and amount of equipment required, then submit completed requisition either on-line or to a HSSC technician. Note BSPN and BSN students must bring their "Nursing Kit" from home as required. As well, all students must bring their clean gloves.
